UNLAWFUL ACQUISITION OF A CONTROLLED SUBSTANCE - She did knowingly and
unlawfully acquire possession of a substance containing Hydrocodone a
controlled substance by misrepresentation, in that she caused to be called
in to CVS, a pharmacy, a fraudulent prescription for a quantity of Vicodin,
a substance containing a schedule II controlled substance Hydrocodone, and
picked up said substance from CVS Pharmacy.

ATTEMPT (UNLAWFUL ACQUISITION OF A CONTROLLED SUBSTANCE) - She with the
intent to commit the offense of unlawful acquisition of a controlled
substance in violation of Illinois Compiled Statutes Chapter 720 Act 570
Section 406(b)(3) performed an act which constitutes a substantial step
toward the commission of that offense in that she knowingly and without
lawful authority came into CVS Pharmacy and requested to obtain a quantity
of Vicodin containing a schedule II controlled substance, Hydrocodone, for
another, knowing she was not lawfully authorized to obtain said controlled
substance.

persons over the age of 21 years with the intent to promote or facilitate

known as Sears with the intent to commit a theft therein.

capacity being the detention of Brock Phillips knowing them to be peace
officers engaged in the execution of their official duties in that he moved
his body in a manner that prevented the officers from handcuffing him.
